We appreciate the opportunity to provide you with quality medical care.  This website has been prepared to help you understand how my office operates.  The more you know, the more we can be of service.  If you have any questions, please ask! We want your visit to be as pleasant as possible.

A good physician-patient relationship is a two-way street. You must feel comfortable about sharing information with me, and our talks are held in strictest confidence.  I must be able to rely on the fact that the information you give me is factual and complete. My ability t practice good medicine and your good health depend on it. Before your visit, be prepared to fully describe how you feel. Have your complete medical history in mind, specific details about your current condition, and all medications you are currently taking. I want to make certain that all of your questions and concerns have been addressed.

DR. KEVIN SCHLAMP is board certified family physician and wound care specialist, practicing in family medicine and industrial medicine. He is a certified Medical Review Officer. Dr. Schlamp also has ten years experience in emergency medicine, 13 years as a local family practice physician and four years as a workers comp consultant.


Appointments may be made by calling the office during regular office hours. In order to allow adequate time, you willl be asked the nature of your visit. If the reason for your visit is urgent in nature and requires a same day visit, one will be made. Because of emergencies, I sometimes run behind schedule. When this happens, my staff will inform you.


Should you find it necessary to change or cancel an appointment, please notify my office within 24-hours. This courtesy will allow someone else to use that available time.


You are encouraged to call with any questions and concerns. My staff or I will call you back as soon as possible with an answer.


When an emergency requires immediate attention, go directly to the hospital ER. If it is during regular clinic hours, please call the office and you will be seen immediately. After regular clinic hours, call the office and the answering service will contact me or the physician on call concerning the emergency.


If you have seen another physician, it is helpful to have his/her previous office records. Consent for release of information must be filled out and signed.


Payments of co-pays, deductibles, or any fees not covered by your insurance company are expected at the time of service.  Cash, check, Visa or MasterCard, may pay fees.  While I expect you to keep our account current, if you are experiencing financial difficulties, please let the office manager know so that we may take it under consideration. I value you as a patient.


We will file all claims to our participating insurance groups. If your insurance carrier is considered out of network, you are required to submit your claim. I encourage you to become familiar with your insurance plan. Please, know where your insurance company is located when it became effective and an employer contact person.


I encourage you to refill all medications during a regular office visit. If this is not possible, please contact your pharmacy and they will notify us via fax. Please allow two business days for all refills.
